Patients should be accompanied by the legal guardian for all appointments.
Our office does not contract with any insurance companies but you can still use your insurance to see us.Payment is expected in full at the time of service. We accept cash, checks and credit/debit /HSA cards. We will give you a superbill that you can submit to your insurance company for reimbursement to you. The amount of reimbursement will depend upon your out of network benefits. Please check with your insurance company accordingly.
The fee for new patient evaluation is $300.
Followup appointments are $100. Followup appointments are generally scheduled once a month. Stable patients may require less frequent followups.
